Before You Begin
Permissions: Ensure your account has permission to access Forms. If you do not see "Forms" in the menu, contact an Admin to edit your Role.
Device Requirement: You must use a desktop or laptop computer. The "Drag & Drop" method for adding fields does not work on mobile devices (Android or iOS).
PayPal Payments: To collect payments (e.g., ticket costs), you must select Registration as the form type.
Image Sizing: If adding a header image, the recommended size is 967px x 300px.
Step-by-Step Instructions
Navigate to Forms in the main menu.
Click the Create New Form button located in the top-right corner.
Select the Type of Form you want to create (e.g., General, Registration).
Note: Select Registration if you intend to collect payments via PayPal.

| Section Description | Provides a description to each section letting users know what data is being requested as well as general information about the form. This field is HTML5 friendly, allowing you to include things such as hyperlinked text, pictures, embed videos, etc. |
Rename your form by clicking on "Untitled Form" at the top.
Important: Do not use quotation marks (" ") in the form field labels or options, as this will break the title.Add a Header Image (Optional).
Click "Add a Header Image" at the top of the editing page.
Select a Tithely-created header or Upload your own (967px x 300px).Click the Create Form button to save your work.
Troubleshooting & FAQs

How do I link form responses to People Profiles? For form data to automatically update a person's profile, there must be an exact match of the following items:
Name & Spelling of the Field.
Type of the Field.
Order of the Options (for Dropdown, Multiple Choice, or Checkbox).
How do I print a blank form? Open the form by clicking View Form, then right-click anywhere on the screen and select Print. You can usually save it as a PDF from the print preview screen.
How do I print or export all responses? Click View Entries under the specific form name. Instead of printing individual responses, click Download as Excel at the top of the page to get all data at once.
Can I attach a document for users to download? You cannot upload a file directly for users to download. However, you can use the Section Description field to insert a shareable link to a document hosted on Google Drive, Dropbox, or a similar service.
Why is my form title broken? Check your field labels and options. Using quotation marks within these areas can break the form title. Remove them to resolve the issue.
