Before You Begin

  • Forms are designed to be shared outside of Tithely. The Forms management area is for Staff/Admin use only and not for filling out forms.
  • Forms cannot be embedded directly into an email—they must be embedded into a website.
  • Embedding forms into WordPress.com-hosted sites may require a workaround due to JavaScript encoding issues.

Step-by-Step Instructions

Sharing a Form (External Options)

1.  Navigate to "Forms."

2.  Click "Share Form" under the form you'd like to share.     

3.  Use the available options:

  • Form Address: The direct link to email or post on social media.
  • Link: Provides a simple hyperlink to embed on your website.
  • Embed: Provides code to embed the form directly onto a webpage.

 

Creating a Shareable QR Code

1.  Visit a QR code generator website

2.  Upload the Form Address URL (the direct link).     

3.  Customize the QR code as desired.

4.  Download the free QR code image for sharing.

 

Sharing a Form with Members Inside of Tithely

1.  Navigate to "Forms" "Share Form" and Copy the Form Address link.

2.  Navigate to Account Settings and select Profile Fields.     

3.  Drag a "New Section" and label it Forms (or similar).

4.  Drag a Section Description into the new section.

5.  For the Section Description, type in the name of the form, paste the shareable form link after a colon and space (e.g., Form Name: URL), and click save. [Members can now access the form via their profile.]

Customization and Troubleshooting

  • Customize Form Address: Edit what comes after (yourchurchname.breezechms.com/form/) by going to Form Settings "Show more options" "Web Address" and adding an easy-to-share label. 
  • Customize Embed Code: The embed code is the most customizable: you can edit the form width, border color, background color, and button color to match your website.
    • Note: If embedding, the Form Title and Description will not be brought into the embed unless you use an iframe embed code (e.g. <iframe src="https://SUBDOMAIN.breezechms.com/FORMURL" style="width: 960px; height: 1250px; border: 1px solid: #fff;" ></iframe> ).
  • WordPress.com Issues: If embedding fails on a WordPress.com-hosted site, it is likely due to the site rejecting JavaScript encoding. Contact WordPress support or use the Direct Website Giving Link method.
